Travel cost support
People claiming certain benefits or on low income are entitled to claim back travel costs to and from hospital appointments. The HC5(T) application form is available from us or can be ordered by calling 0300 330 1343.
Complaints
If you are not happy with the service or care you have received at Tetbury Hospital Trust, please speak to a member of staff. If you are not happy with what they tell you, ask to speak to someone senior such as a department manager, a matron or your consultant. They will try and resolve your problems straight away or will direct you to someone else who can help. You can also contact us on 01666 502336 or fill in this Complaints form and email it back to us.
